What we are looking for:

We are looking for a passionate and proactive Liaison & Diversion Practitioner to support service users on the Isle of Wight. This is a highly administrative role in which, you will be required to ensure service users successfully achieve their planned objectives, by delivering effective interventions and taking a multi-agency approach to service delivery ensuring the right services are available. These include mental health, primary care and substance misuse services, alongside other support services such as family services, positive activities, early interventions support and wider community groups.

This successful candidate will need to demonstrable experience of working with vulnerable members of society in a supportive role. Coupled with this work, the role will require extensive administration, so a high level of IT literacy is required to ensure that front line delivery meets the contract specification and is delivered to a high standard.

Duties and Responsibilities involve but are not limited to:

Provide information advice and guidance, against multiple pathways and supporting with the signposting of service users to divert them out of the criminal justice system.
Delivering a high level of IT skills to ensure recording is completed across different systems, along with creating criminal justice reports for police and courts 
Assessing Service Users needs (using assessment proforma) Linking service users to appropriate  support services (by following an administrative process)
Supporting service users to attend initial appointment when necessary     
Risk assess and take the necessary actions in respect of any activities / engagement with service users including personal safety.
Actively engage with service users in decisions that affect them, informing them of their procedural and representational rights, ensuring they have input in shaping the nature of services delivered to them through involvement activities ranging from providing information, requesting feedback, consulting, to facilitating participation by the service users.
Maintain contact on a regular basis with service users in accordance with the requirements of the service. This includes maintaining a professional relationship and being a good role model to service users.
