Job Description
This is a hybrid role (In-Office & Out-of-Office work). The RN Case Manager reports directly to a Clinical Supervisor and is responsible for care coordination of clinical staff, patients and their families/caregivers, and physicians or other care providers. Responsible for admissions, recertification assessments, transfers/resumptions of care, and discharge assessments, including supporting documentation. This position ensures compliance with MGA policies, state policies and procedures, and policies of accrediting agencies, and maintenance of patient confidentiality/HIPPA guidelines. MGA provides a cohesive interdisciplinary team that prioritizes patient care.
Duties and Responsibilities   
Ensures direct supervision of CNAs, LPNs, and RNs in the field. This involves training and re-education of staff on clinical skills/procedures when necessary.
Develops an individualized plan of care for each client.
Ensures effective communication to coordinate the implementation of each patient’s Plan of Care.
Provides support, guidance, and education to patients and their families/caregivers to assist patients in their progress toward their goals.
Completes all clinical documentation in a timely and accurate manner, and ensures all documentation meets the requirements of agency policies and state regulations.
Reviews documentation of peers and field staff to ensure accuracy to meet payer source requirements for reimbursement, and provides re-education as needed.
Escalates any concerns to the Clinical Supervisor/Director of Clinical Services. Collaborates with Clinical Supervisor and Director of Clinical Services to resolve client/clinical concerns in timely manner.
Complies with and participates in continuing quality improvement by reporting incidents, injuries, and infections.
Interviews and evaluates potential clinical field staff to determine competency and fit for our clientele.
Supports and executes the mission, ethics, and goals of the company effectively.Represents themselves in a positive and professional manner in the company and community.
Adheres to dress code with a clean and neat professional appearance.
Reports on time and as scheduled in order to complete work within designated time.
Adheres to all company policies and procedures outlined in Employee Handbook, Employee Agreement, or communicated from executive team.
